Error When Importing Feedback from Google Sheets

I attempted to import some feedback I'd collected in Google Sheets but received the error:

It's not clear what I need to change to make it work correctly. I used the template provided in the instructions. The only things missing are the user names/emails and date fields, since I collected this myself. There are only 36 entries. I tried both CSV and XLSX. The error message might need to be clearer for the user to understand what they need to change.
